Your First Appointment

Arrival:
At your first appointment, it is essential that you arrive 15 minutes prior to your scheduled time so you may fill out the patient information forms. If you arrive more than 15 minutes late, you will be rescheduled. Below, please find a listing of our forms; the first two are needed for all patients who are seeing us for the first time.

Downloadable forms:

Appointments and Cancellations

We know your time is valuable and we diligently try to stay on schedule. On some occasions, emergencies do occur and appointment schedules may be altered. We will inform you as soon as possible should this happen. Your understanding is greatly appreciated. If you are unable to keep your appointment, please give our office 24-hours notice, as we have a long waiting list and wish to accommodate everyone as fairly as we possibly can.

Referrals:
You MUST have your referral with you--if required by your insurance--when you arrive to your appointment.
Otherwise, you will need to sign a waiver assuming full financial responsibility for your visit. Please remember that we cannot schedule further testing or follow-up appointments unless you have your referral. This is the requirement of almost all insurance carriers.

Films, Reports, Medications:
It is also essential that you bring the appropriate films and written reports from your primary care physician (your regular doctor). These items include MRI scans, x-rays, CT scans, EMGs, or other pertinent test.

If you take any medications, please bring your pill bottles with you.

Please check with your primary care physician and make sure that you have these items with you on your first visit to our practice. The neurologists from our practice may not be able to answer your questions if you do not bring the appropriate films and reports when you come for your visit.

Payment and Insurance

Payment:
Upstate Neurology Consultants accepts VISA, MasterCard, American Express, and Discover, as well as cash (exact amount, please) and personal check. Payment for services (including co-payments and co-insurance) is expected at the time of your office visit.

Insurance:
We participate in many major insurance plans in the region including Medicare, CDPHP, Mohawk Valley Physicians, Empire Blue Cross and Blue Shield, NYS Empire Plan, Blue Shield of NENY, Aetna, and No-Fault. We do not treat Workers' Compensation patients. If you have a question as to whether our physicians participate with your plan, please call your insurer. It is the patient's responsibility to know their insurance contract and to follow-up on denied or delinquent claims. Your co-payment and/or coinsurance is due at the time of service.

No-Fault Insurance Requirements:
If you have a No-Fault claim, you MUST inform our office at the time the appointment is made so that we can mail you necessary paperwork. You must complete the required forms prior to your appointment and mail them to our office 7 days prior to your appointment. Failure to do this will result in your appointment being rescheduled. (See list of forms above.)

Prescriptions and Renewals:

Please call the office if you have a problem with your medication. If you need a renewal of your current prescription, please call at least three business days in advance of running out of the medicine. We have an automated refill line to speed your refill, including triplicates and 90 day refills. Dial our direct automated refill line at 518.533.1561. You will need to know the name of the prescription, strength, and dosage as well as the pharmacy name and telephone number before calling.

You can call in your prescription renewal request any time, 7 days a week. Prescriptions will be filled by the next business day.

No pain or headache medications will be prescribed at nights or on weekends. If you are in sufficient pain that you need medication, then you will need to go to the emergency room to be evaluated.
